Across multiple clinic locations, integration of comprehensive geriatric assessments into oncology workflows, real-time data tracking, multidisciplinary team coordination, and dedicated support services allowed providers to address the unique needs older patients. Outcomes from this model program show 78% of patients assessed had functional impairments, 562 referrals to supportive care services, and a significant increase in advance care planning and clinical trial enrollment.
